内容摘要:After several decades of clinical application, the use of radiotherapy to most common cancers has become a standard management with a list of clinically verified protocols. But radiotherapy in its early era caused many complications to patients, because it could not prevent the damage to normal structures. With the introduction of new techniques in recent ten years, now we can spare more and more normal or critical structures while delivering treatment dose to tumors. In addition, new machine and techniques are able to adapt organ’s motion and can track tumor movement during the treatment. Stereotactic techniques, such as Stereo-Body Radiotherapy (SBRT) and Stereo-Radio-Surgery (SRS) with the concepts taken from Gammaknife, are another approach to pin-point tumor and avoid harming normal tissues. With the advanced body-immobilization and stereotactic techniques, now we are able to use larger dose to more accurate tumor locations without causing the rise of severe complications than ever before. Brachytherapy now has been used in almost all tumor sites in high dose rate and low dose rate modes, the eye-plaque brachytherapy to eye melanomas, mesh-brachytherapy to early stage non-small cell lung cancers and liver cancers have significantly improved patient’s survival rate and quality of life. On research side, more and more physicists and physicians are focusing on using all kinds of radio-biology modeling methods to explain and design treatment regimens and even outcomes. In this talk, I will introduce all available radiotherapy modalities in US, and discuss possible research directions which may potentially change or rewrite cancer treatment outcomes.
